Privacy Policy

Our facility is committed to providing quality heath care for its patients, along with best practice in relation to management of information we collect. This practice has developed a policy to protect patient privacy in compliance with privacy legislation.

While you are a patient under our care, we need to collect personal information to store within your medical record. This information may include any or all of the following-

  • Name and demographic details
  • Medicare and health fund details
  • Medical history, reports or results of investigations/tests
  • Notes made during consultation
  • Clinical notes from other practitioners

This information may be obtained from you directly, from a person responsible for you or by third parties where the National Privacy Act allows it. This information will be used to provide health services to you, to communicate with you, to assist in managing our administrative accounts and services, and to comply with legal obligations.

Any and all information obtained for your medical records are kept away from the general public and are kept in a locked facility during out of office hours. Any medical information obtainable electronically is user ID and password protected at all times. Any IT personal that may have access to the medical record system/s and information are covered by the National Privacy Act and are under strict confidentiality agreements with Cancer Fit Australia PTY Ltd.

All staff who have access to your medical records are bound by a strict confidentiality agreement as part of the terms of their employment. Any personal information handled by staff will be handled with the utmost respect for your privacy.

Your medical records will not be released without permission, except for special circumstances such, but not limited to, an emergency or subpoena. This will be determined on a case by case basis by your medical practitioner.

You may not take your medical records off site, make any corrections or remove any information from your patient records, however, you are able to add any additional information of relevance.

As a patient, it is within your rights to have access to any information we hold concerning you. You may seek access to this information with a written request. Once this request has been submitted to CANCER FIT AUSTRALIA PTY LTD, your request will be processed and you will be advised of the outcome. If a fee is charged for access to your records, you will be advised of the cost in advance.

CANCER FIT AUSTRALIA PTY LTD take all reasonable steps to ensure our patients privacy and the security of all information. If you have any questions or concerns about privacy related issues, or wish to complain about a breach of the Australian Privacy Principles or handling of your personal information by us, please speak to the manager or to your treating physician.

This policy will be reviewed and updated from time to time in account of instances such as new laws, changes in technology and changes to our operations. and If you have any questions in relation to our Privacy Policy, please speak with the manager or your treating physician for further information.
